If you are someone who needs assistance with your day-to-day activities and are Medicaid eligible, you can enroll in the New York State Consumer Directed Personal Assistant Program (CDPAP) today!

CDPAP is a unique Medicaid program that gives the elderly and disabled the ability to manage their care and make decisions based on their personal needs.

This program allows the elderly or disabled to reclaim independence by living at home. The patient maintains total freedom of choice in setting up a care schedule and directing how and when their plan of care is managed. They also have the ability to choose their own caregiver. Caregivers can be an immediate family member, friend, relative or neighbor, as long as they are at least 18 years old.

REQUIREMENTS TO JOIN PROGRAM:

1. Patient must be enrolled in Medicaid to receive CDPAP services.
2. Patient must live in the state of New York.
3. Both the patient and caregiver must be at least 18 years old.
4. Caregiver must be a legal resident of the United States.
